FEDERAL APPLICATION
Dear Applicant,
The roles of DMAT's may vary thus team configuration and the level of medical expertise required of team members will be appropriate to the mission. All Physicians, PA's/ARNP's, RN's and Paramedics must be ACLS and BLS certified. All other personnel must be BLS certified and First Responder qualified as a minimum. If deployed the team will remain on active status for up to two (2) weeks.
Please familiarize yourself with the South Florida Regional DMAT teams standards for application. A minimum amount of volunteer training exercises is mandatory. When you are ready to submit your application, please contact the team Admin Officer at debfl5admin@bellsouth.net or bring your paperwork to our next recruitment meeting.
Print the following forms needed to apply for application to the South Florida Disaster Medical Assistance Team (D.M.A.T.) FL-5. Leave item number two (#2), grade applying for blank on the OF-612 form; the Administrative Officer will fill it in. Once you have completed the forms, you will need to make two (2) copies, including copies of all of your licenses and certifications as well as your drivers license, social security card and passport if available. Bring both copies to the Monthly recruitment meeting along with your drivers license and social security card or passport (if not available) . These meetings are held at location to be announced of every month. Once there you will be sworn in, have a digital photo taken, and have your application checked for completeness.
A panel review of the applicant may consist of observation of skills, request for references, or additional information to determine that the applicant meets the standards established. After approval by the panel, the application will be forwarded to FEMA for processing. This generally takes several months. During the application process FEMA they will conduct a background check which will include finger printing and a FBI background check.
You may attend any of the monthly general meetings, however you cannot participate in any of the deployments or drills until we have received notification of your approval from FEMA. We will NOT be processing any applications during the General Meetings.
