View your data and if anything has changed, goto the member update form and resubmit your personal data. Just check the "Other" box as the charity of choice and write in "The South Florida Regional Disaster Medical Assistance Team" Your monies will then be mailed to us, that's all there is to it; so let your family, friends, and co-workers know the FL-5 is the charity that they should choose for the United Way!!!
